=== Mechanism of action === Phentolamine acts as a reversible dual or non-selective α1- and α2-adrenergic receptor antagonist. Its primary action is vasodilation due to α1-adrenergic receptor blockade. Reportedly, phentolamine may also stimulate β-adrenergic receptors. Non-selective α-blockers can cause a much more pronounced reflex tachycardia than the selective α1 blockers. Like the selective α1 blockers, phentolamine causes a relaxation of systemic vasculature, leading to hypotension. This hypotension is sensed by the baroreceptor reflex, which results in increased sympathetic nerve firing on the heart, releasing norepinephrine. In response, the β1 adrenergic receptors on the heart increase their rate, contractility, and dromotropy, which help to offset the decrease in systemic blood pressure. Unlike the α1 selective blockers, phentolamine also inhibits the α2 receptors, which function predominantly as presynaptic negative feedback for norepinephrine release. By abolishing this negative feedback phentolamine leads to even less regulated norepinephrine release, which results in a more drastic increase in heart rate.
=== FDA approval === In 1988, twenty-six years after the 1962 introduction of breast implants filled with silicone gel, the U.S. Food and Drug Administration (FDA) investigated breast implant failures and the subsequent complications, and re-classified breast implant devices as Class III medical devices, and required from manufacturers the documentary data substantiating the safety and efficacy of their breast implant devices. In 1992, the FDA placed silicone-gel breast implants in moratorium in the U.S., because there was "inadequate information to demonstrate that breast implants were safe and effective". Nonetheless, medical access to silicone-gel breast implant devices continued for clinical studies of post-mastectomy breast reconstruction, the correction of congenital deformities, and the replacement of ruptured silicone-gel implants. The FDA required from the manufacturers the clinical trial data, and permitted their providing breast implants to the breast augmentation patients for the statistical studies required by the U.S. Food and Drug Administration. === Civil and criminal proceedings === In March 2018, the US Securities and Exchange Commission filed civil fraud charges against Theranos, its CEO Holmes and former president Ramesh "Sunny" Balwani, claiming they had engaged in an "elaborate, years-long fraud" wherein they "deceived investors into believing that its key product – a portable blood analyzer – could conduct comprehensive blood tests from finger drops of blood". Holmes reached a settlement with the SEC which required her to pay $500,000, forfeit 19 million shares of company stock, and be barred from having a leadership position in any public company for ten years. Balwani did not settle with the SEC. On June 15, 2018, Holmes and Balwani were indicted on multiple counts of wire fraud and conspiracy to commit wire fraud. According to the indictment, investors, doctors and patients were defrauded. It is alleged that the defendants were aware of the unreliability and inaccuracy of their products, but concealed that information. The case, United States v. Elizabeth A. Holmes, et al., was assigned to Lucy H. Koh, United States district judge of the United States District Court for the Northern District of California. This is significant because a urine immunoassay drug screen typically tests for morphine, which is a metabolite of a number of legal and illegal opioids such as codeine, morphine sulfate, and heroin. Trace amounts of 6-MAM are excreted approximately 6–8 hours following heroin use. 6-MAM is naturally found in trace amounts in rat and cow brains.
